At Longreach State High School the Tier II PBL Team includes relevant stakeholders including the Principal, Deputy, Head of Departments, Youth Support Officer,  Guidance Officer, HOD Inclusion and Schoob Based Youth Nurse. This team meets fortnightly.

The aim/purpose of the student services meeting is:

  • To identify and organise support for students 'at risk' of disconnecting with schooling or who may be struggling with personal issues

  • Work as part of a multidisciplinary team and facilitate effective working relationships and partnerships with parents, school personnel and external support agencies in order to provide a comprehensive support, case-management and referral service that optimises students' access and engagement in educational programs

  • Provide a counselling and referral service to assist students in decision making about critical educational, personal, social, emotional and/or career development, and provide ongoing support during the implementation phase of their decision

  • Conduct activities such as student observation and psycho-educational assessment in order to determine the nature of student learning difficulties, disabilities, developmental levels or psychological and emotional status in order to make recommendations for educational adjustments and interventions

  • Assist schools in the implementation of student protection, gifted education, behaviour support policies and risk management processes.

Such documentation that may be implemented to identified students are:

  • Education Support Plans (ESP) to cater for students that are in care of the state with assistance with the carers/teachers and child safety officers
  • Support Plan to cater for students to improve their mental health/well-being (self-harm/suicide ideations) etc
  • Individual Behaviour Plan to cater for students in order to minimise misbehaviour and improve and manage own behaviour (FBA)
  • Risk Management Plan to cater for students to reduce risk to themselves and others
  • Individual Learning Plan / Individual Curriculum Plan to cater for students learning needs by focusing on their strengths and eliminating their barriers in order to best achieve the Australian Achievement Standards
  • Flexible Learning Plan – cater for student's workload depending on individual circumstances (work experience, apprenticeship, traineeship, mental health well-being, unsafe behaviours etc)
  • Behaviour Application Form – to cater for students with complex learning and behavioural needs and to receive extra teacher aide funding/time
  • Complex Case Management Plan – to cater for student's diverse needs in the best possible way; medical, behaviour, disability, emotional control etc.
  • SET Plan – to cater for subject selection for students that align with their career aspirations
  • Cognitive Testing (WISC-IV/WPPSI-IV/ABAS-3) – allows to identify for Intellectual Disability, specific Learning Difficulties and Giftedness. Reports provide quality recommendations to staff/parents while maintaining confidentiality and ethical standards.

In order for these plans to be effectively established the school must work alongside with external agencies.
